Q - How does it work?

Simply locate a network provider and present your membership card at the time of service to receive instant discounts. That's it! The Formulated Dental Choice Plan is not insurance, so you don't have to worry about paperwork. Discounts are received immediately at time services are rendered.

Q - Is dental work started before joining the program discounted?

Dental work STARTED before signing up for the dental plan is NOT, even if completed after joining. However, existing dental problems ARE discounted, when work is started after joining.
